The challenge
Every new patient at Meridian meant forms keyed in by hand, insurance details verified over the phone, and records copied between three systems. Front-desk teams spent more time on data entry than on patients, and a single typo could delay care by days.
Compliance made it worse: every step needed an audit trail, so even small fixes had been off-limits for years.
What we built
We mapped all 14 steps, then rebuilt the flow as an automated pipeline: documents are parsed by a language model, insurance eligibility is checked against payer APIs, and records sync to the EHR through a FHIR integration.
Anything ambiguous routes to a human review queue with full context attached — staff approve edge cases in seconds instead of re-keying entire files. Every action is logged for audit.
